Carrier Access and Asphalt Product Loading
Agreement for the Marshall Avenue Facility

If you will be bringing your own transportation equipment to load asphalt products at our facility, you will need to sign this agreement and return to U.S. Oil and to meet the insurance requirements specified in the agreement. Your drivers will also be required to complete training on the facility's operating rules and procedures. This training can be scheduled with the Scalehouse at (253) 383-1651 after the signed agreement and insurance certificate have been received by U.S. Oil.

Department of Transportation, 49 CFR PART 172 -
Compliance Statement

The Department of Transportation (DOT) has issued regulations concerning the security of hazardous materials transported in commerce. The regulation requires shippers and carriers of certain highly hazardous materials to develop and implement security plans and assure that their employee-training program includes a security component. Some of these hazardous materials are transported to and from our facility.

In order to comply with DOT regulations, U.S. Oil Trading LLC requires all of its carriers to complete a compliance statement stating that the carrier has developed and implemented a security plan in accordance with DOT 49 CFR Part 172.

If the material being carried does not fall into the hazardous materials classification, please check the appropriate box and sign the compliance statement. Please contact, via e-mail, at deb@usor.com if you have any questions.

Carrier Authorization
If you use a common carrier and/or you are your own carrier, you will need to fill out and sign this authorization form to give us the authorization of who could load for your account. The carriers you authorize will have to meet the same requirements specified above for bringing transportation equipment to load asphalt precuts at our facility.
